Have you travelled overseas in the past 14 days? Do you feel unwell? Including, but not limited to, symptoms of COVID-19 such as fever, cough, sore throat or shortness of breath?
Have you had any contact with a confirmed or suspected case of COVID-19 in the past 14 days? Measures have also been put in place to minimise the risk and spread to our patients and staff:
Our staff are trained in comprehensive and appropriate medical grade infection control procedures and will continue to uphold our high clinical and sanitation standards.

Patients are required to use hand sanitiser provided before and after their appointment.
